29 CFR  1910.147(d):Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  (2) turn off and shut down the machine  (3) physically locate and operate the energy isolating devices in such a manner as to isolate the machine or equipment from the energy sources. (4) affix lockout or tagout devices to each energy isolating device  (5) Following the application of lockout or tagout devices to energy isolating devices, all potentially hazardous stored or residual energy shall be relieved, disconnected, restrained, and otherwise rendered safe.  (6) Prior to starting work on machines that have been locked out or tagged out, verify that isolation and deenergizing of the machine or equipment have been accomplished.   (a) On or about 12/14/2020 the employer failed to implement energy control procedures in that the Coater #4 (model #OPG-LRB) was not shut down nor shutoff to perform the servicing work such as but not limited to: cleaning of the rollers [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i), and verification of isolation [(d)(6)], were not implemented to protect employees from machine hazards such as an employee reaching into the Coater to remove excess coating, while the machine was running at 350 ft. per min. exposing the employee to a caught in hazard.

Safety-related work practices shall be employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work is performed near or on equipment or circuits which are or may be energized. The specific safety-related work practices shall be consistent with the nature and extent of the associated electrical hazards.  On or before 12/14/2020 the employer did not ensure that authorized employees performing maintenance on the Coater #4 (model #OPG-LRB) were following Safety-related work practices.  Authorized employees were exposed to 240 volts while locking out of the machine due to the lock out procedures requiring them to open the 240 volt electrical cabinet, reach into the cabinet and switch two breakers off while the rest of the wiring and fuses were live.
