Main Street, Norwalk, Ohio 44857: On or about December 21, 2020, the employer did not provide a medical evaluation to determine each employee's ability to use a respirator before requiring the use. The employer required employees to wear respirators while providing care to suspected and confirmed positive COVID-19 residents.

29 CFR 1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested whenever a different respirator facepiece (size, style, model or make) was used: a) Twilight Gardens Nursing & Rehabilitation located at 196 W. Main Street, Norwalk, Ohio 44857: On or about December 21, the employer permitted the use of a N95 respirator [Makrite/9500-N95 84A-5411] after an employee failed the fit test and was not retested with other respirator make(s), model(s), or size(s) to assure a proper fit. The employer required the employees to wear N95 filtering facepiece respirators to protect against the SARS-CoV-2 virus while providing care to suspected and confirmed positive COVID-19 residents.
