29 CFR  1926.403(b)(1):Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:  (a) Jobsite located at 5700 Market St. Philadelphia, PA - 19139: Employees were exposed to energized conductors in that the employer did not ensure that correct adapters were used to energize two flexible power cords. The employer allowed copper wiring to be inserted into the auxiliary power receptacles of the welding unit and attached to the hot and neutral prongs of the 3-pronged flexible power cords on two connecting devices, thus exposing the employees to electric shock hazards.  Condition was observed on or about December 21, 2020.
