29 CFR 1926.55(a)(2): Employee(s) were exposed to an airborne concentration of carbon monoxide listed in Table 1 in excess of the 8-hour time-weighted average concentration of 50 parts per million:   On or about 12/15/2020,  the employer exposed employees to airborne concentration of Carbon Monoxide of 499 parts per million in excess of the 8-hour time-weighted average concentration of 50 parts per million while operating propane powered equipment such as, but not limited to Whiteman concrete polishing machine (Model# HHXDF5 & Serial# MJ0211389).
