29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   Sierra Place Apartments, Building #1 roof over unit #201: On or about 12/18/2020, the employer exposed employees to falls of over 17-feet, 5-inches from the lower level, in that employees were not connected to a fall protection system while engaged in roofing activities at the gable edge on a 4/12 pitched roof.

29 CFR  1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.(a)(LOCATION)(IDENTIFY SPECIFIC OPERATIONS(S) AND/OR CONDITIONS)   Sierra Place Apartments, Front side of Building #1 : On or about 12/18/2020, the employer exposed employees to potential fall hazards of up to 17-feet, 5-inches while accessing a roof using a ladder which was damaged, in that the ladder was not removed from service as the feet of the ladder were missing and the side rails were sinking into the soil exposing employees to falls from a ladder failure and potential tip-over hazard.
