29 CFR 1910.132(f)(1)(ii):The employer did not provide training to each employee who is required by this section to use personal protective equipment (PPE) on knowing what PPE is necessary: (a) Berk International, 400 East 2nd Street, Boyertown, Pennsylvania - Employees performed work activities, including, but not limited to, servicing the bandsaw on the Lyish LFN 100 Wipes Napkin Folding Machine (Line #9), exposing the employees to the hazard of contact with the bandsaw blade, and the employer did not provide all employees with training on the requirement to wear metal mesh gloves, on or about December 9, 2020. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: (a) Berk International, 400 East 2nd Street, Boyertown, Pennsylvania - An employee suffered an amputation to the right middle fingertip on December 9, 2020 as a result of contacting a stationary bandsaw blade. The employer failed to report this work-related amputation to OSHA within 24 hours. (The employer reported this amputation to OSHA on December 14, 2020.) No abatement certification required.
