29 CFR 1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested prior to initial use of the respirator and were not tested annually: a) Emergency Department: On or about April 7, 2020 : Employees were not fit tested for initial and annual N95 use while performing patient care duties. Employees treated COVID 19 + patients in the Emergency Department and did not have fit tests completed annually. The Department of Veterans Affairs was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.134(f)(2), which was contained in OSHA inspection number 1116115, notice number 1, item number 1 and was affirmed as a final order on 6/8/2016, with respect to a workplace located at 800 Zorn Ave, Louisville, KY. The Department of Veterans Affairs was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.134(f)(2), which was contained in OSHA inspection number 1140139, notice number 1, item number 1 and was affirmed as a final order on 6/6/2016, with respect to a workplace located at 3801 Miranda Ave. Palo Alto, CA.
