29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables used for temporary wiring were not protected from damage: On or about December 21, 2020, two employees were performing welding tasks in two separate aerial lifts that were being driven over temporary extension cords that were not protected from damage, exposing employees to hazards such as, but not limited to electrical shock, burns or fire.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift. On or about December 21, 2020, two employees were performing welding tasks in an aerial lift up to approximately 12 feet high without a harness and lanyard securing them in the basket, exposing the employees to a fall hazard.
