29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) On or about December 22, 2020, the portable ladder being used for access to the roof did not extend at least 3 feet above the upper landing area.  The WA Home Improvement LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1234005, citation number 2, item number 1 and was affirmed as a final order on November 8, 2017, with respect to a workplace located 13988 West Lindfield Drive, New Berlin, WI 32151.

29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a)  On or about December 22, 2020, employees engaged in re-roof of a restaurant on a 6/12 pitched roof, were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 10 feet.  The WA Home Improvement LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1234005, citation number 2, item number 1 and was affirmed as a final order on November 8, 2017, with respect to a workplace located 13988 West Lindfield Drive, New Berlin, WI 32151.  The WA Home Improvements LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1100882, citation number 1, item number 2 and was affirmed as a final order on June 30, 2016, with respect to a workplace located at 3402 West College Avenue, Greenfield, WI 53221.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a)  On or about December 22, 2020, the employer did not ensure that employees exposed to potential fall hazards had been trained to recognize and minimize the fall hazards associated with their work.
