29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) 8957 U.S. 9, Chazy, NY- On or about December 12, 2020, an employee was injured while conducting the removal of chicken cages, which resulted in a hospitalization for a period of several days. The employer was notified of the hospitalization and failed to report it to OSHA within a 24 hour reporting period.
