29 CFR 1910.28(b)(3)(ii): Each employee was not protected from tripping into or stepping into or through any hole that was less than 4 feet (1.2 m) above a lower level by covers or guardrail systems: Employees engaged in housekeeping, and grain handling activities in the distribution floor of the grain elevator were exposed to tripping and fall hazards into opening floor hole of 16 inches. The employer did not assure covers were readily available and installed for all floor holes. There were 7 open distributor holes but only 3 covers available, they were missing 4 covers. Employees were permitted to walk and work near and around floor holes that did not have covers installed. 29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.
