29 CFR  1910.134(e)(3)(i): The employer did not provide a follow-up medical examination for employees who gave a positive response to any question among questions 1 through 8 in Section 2, Part A of Appendix C, or whose initial medical evaluation demonstrated a need for a follow-up medical examination:   a) Sunbridge Gardendale Health Care Center, LLC dba Genesis Healthcare - Magnolia Ridge Center located at 420 Dean Drive, Gardendale, Alabama:  On or about December 26, 2020, the employer did not ensure that a follow-up examination was provided to employee(s) who gave positive responses to the medical questionnaire. The employer required employees to wear respirators while providing care to suspected and confirmed positive COVID-19 residents.

29 CFR  1910.134(e)(6)(i): The employer did not obtain from the physician or other licensed health care professional (PLHCP)  a written recommendation regarding the employee's ability to use the respirator:  a) Sunbridge Gardendale Health Care Center, LLC dba Genesis Healthcare - Magnolia Ridge Center located at 420 Dean Drive, Gardendale, Alabama:  On or about December 26, 2020, the employer did not obtain a written recommendation by a physician or other licensed health care professional regarding the employee's ability to use respirators. The employer required employees to wear respirators while providing care to suspected and confirmed positive COVID-19 residents.
