29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition:  (a) In the production area near the Selco Baler; Explosive dust accumulation was present on surfaces in several areas including but not limited to, the floor near the southeast wall, bales of processed mixed paper product and other recycled material, and overhead structural support beams.

The door was obstructed, locked, and not intended to be used as an emergency exit.
