29 CFR 1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice. On or about December 29, 2020 and at times prior thereto, the employer did not ensure that the walking-working surface of the catwalk floor inside the covered drum room near the delivery shoot was maintained free from hazards such as corrosion. This exposed employees a fall hazard.

29 CFR 1910.28(b)(3)(v): The employer did not ensure that each employee is protected from falling through a hatchway and chute floor hole by one of the methods listed in 29 CFR 1910.28(b)(v)(A)-(C): On or about December 29, 2020 and at times prior thereto, the employer did not ensure that employees were protected from falling through a hatchway that is approximately 4 feet by 4 feet. This exposed the employees to a fall hazard greater than four (4) feet to the concrete below. a. Th hatchway cover was removed for employees to move large pieces of concrete. The employees were not in use of fall protection. b. The hatchway was left uncovered after clean up and an employee walking fell through the hatchway.

On or about December 29, 2020 and at times prior thereto, the employer did not ensure that employees, walking/working below the catwalk floor inside the covered drum room, were protected from falling objects. The employees are exposed to struck-by hazards.
