29 CFR 1926.451(g)(1)(vii):Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. On or about December 24, 2020 south side of the building employees were observed working off a homemade scaffold attached to the building approx. fifteen (15) off the ground without the use of a personal fall arrest system.

On or about October 20, 2020, on the south side of the building an employee was observed using a framing nail gun to attach plywood to the building without any personal protective equipment (PPE) such as safety glasses.
