29 CFR 1910.178(m)(5)(iii): Powered industrial truck(s), with the operator dismounted and within 25 feet of the truck still in his view, did not have the brakes set: a) 1203 Kinne Street, East Syracuse, NY 13057: On or about 12/30/2021, an employee placed a trailer at the loading dock and dismounted the cab for the purpose of placing a jack stand under the trailer. The employee did not engage the air brake and the tractor subsequently rolled backward to the trailer, trapping the employee between the 5th wheel connection. This employee subsequently died.
