29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): A) On or about January 5, 2021 at the 6 Church Hill Road, Wethersfield, CT job site: Employees were observed removing roof shingles from the eastern section of the house roof with slope of approximately 8:12. The fall hazard to the ground below was approximately about 8 feet from that area to the ground below. Employees were not protected by a personal fall arrest system or other means of fall protection.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: A) On or about January 5, 2021 at the eastern corner, 6 Church Hill Road, Wethersfield, CT job site: The employer did not ensure that the 16-foot aluminum extension ladder used to access the roof surface extended at least three feet past the landing surface. This was a slipping and fall hazard to the employees who traveled the ladder up and down carrying roofing materials.
