At the residence On or about 1/5/2021,  employees were engaged in roofing activities on an approximate 6/12 pitched roof, approximately 20-feet above the lower level without fall protection.

At the worksite On or about 1/5/2021, employees were exposed to fall hazards from ascending/descending an extension ladder that was leaning against the downward slope of a roof rake edge, preventing equal support to both ladder rails.
