29 CFR  1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  (a)   On or about January 6, 2021, employees installing siding from their pump jack scaffold on a new residential property were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 16 1/2 feet.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  (a)  On or about January 6, 2021, employees were using the roof to access their pump jack scaffold on a new residential property with a 9/12 pitched roof, were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 11 feet.

The employer did not ensure that employees exposed to potential fall hazards had been trained to recognize and minimize the fall hazards associated with their work.
