29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented, and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Euromac Press - Model CX1000/30, Serial #3MM324903A: On or about November 09, 2020, the employer did not insure lockout procedures were utilized by employee(s) when substantial adjustments and die changes were completed as part of machine setup. The machine was energized while employee was conducting die changes, exposing the employee(s) to ingoing nip point, crushing hazard and amputation injury.

29 CFR 1910.147(f)(1)(ii): Employees were not removed from the machine or equipment area in accordance with paragraph (e)(2) of this section during positioning of machines, equipment or components thereof. (a) On or about November 09, 2020, employee(s) working on and around the Euromac model CX1000/30 (Serial number #3MM324903A), while conducting setup and tool change, were not sufficiently removed from the point of operation while the machine was returning to the down position as a part of the completion of setup. This exposed the employee to a caught-in/crushing hazard and amputation injury. In the alternative... 1910.212(a)(1): Machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts and crushing hazards. (a) On or about November 09, 2020, employee(s) working on and around the Euromac model CX1000/30 (Serial #3MM324903A), while conducting setup and tool change, were not sufficiently guarded from the point of operation while the machine was returning to the down position as a part of the completion of setup. This exposed the employee to a caught-in/crushing hazard and amputation injury.
