29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) On or about December 29, 2020 in the meat department, employees performing servicing and/or maintenance such as but not limited to cleaning the saw blades and scrap tray of the Hobart Model 6614 Meat Saw, ML-134096, SN:311553701 when specific lockout and tagout procedures were not documented and utilized nor was it unplugged. This condition exposed employees to caught in and struck by hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about December 29, 2020 the employer failed to ensure the unused portion of the blade on the Hobart Model 6614 Meat Saw, ML-134096, SN:311553701, in the meat department, was guarded when employees used the saw to cut meat exposing employees to caught in and struck by hazards.

29 CFR 1910.138(a):The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes. (a) Employees operating equipment including, but not limited to Hobart meat saws were exposed to struck by and caught in hazards because they were not provided adequate hand protection.
