29 CFR 1926.453(b)(2)(v):A personal fall arrest system was not worn with a lanyard attached to the boom or basket when working from an aerial lift. Site - On or about January 5, 2021, employees working from the basket of a JLG 400S telescopic boom lift were not protected from a fall of approximately 10 feet where they did not have a lanyard properly attached to the boom or the anchor point in the basket of the lift. Construction Ahead Exteriors Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1320154, citation number 1, item number 2 and was affirmed as a final order on November 21, 2018, with respect to a workplace located at 3090 West Main Street, St. Charles, IL 60175.

29 CFR 1926.20(b)(2): Employer did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. Site - On or about January 5, 2021, the employer did not perform regular job site inspections where employees were exposed to fall hazards when their lanyards were not connected to the telescopic boom lift anchor points, within view of the foreman on site.
