29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers: a) Boyer Hill Military Housing at 4330-A Washington Street, Hill AFB, UT, 84056: On or about January 5, 2021, the employer did not ensure that each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers. Employees were removing flooring to be replaced inside a residence. This condition exposed employees to trip and fall hazards and led to the injury of an employee that fell into an approx. 2 ft. by 2 ft. hole in the floor.
