29 CFR  OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment in a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees that were exposed to thermal and slip/fall hazards.  These hazards were caused by the use of unsuitable components in the drain line of a conical mixer during cleaning/sanitizing operations:   Compounding Area, Nauta 5 Mixer:  On January 7, 2021, a ClearFLEX tube/hose was used as part of the discharge line from the mixer system during the sanitizing cycle. The pressurized, heated sanitizing water was allowed to enter the hose for the duration of the sanitizing cycle due to the placement of the discharge valve downstream of the hose.   the temperature of the water exceeded the working range of the hose.  A hose clamp failed and the heated water was released from the system, seriously injuring an employee.
