29 CFR 1910.36(b)(2): More than two exit routes were not available in the workplace where the number of employees, the size of the building, its occupancy, or the arrangement of the workplace was such that all employees would not be able to evacuate safely during an emergency: Second Floor Warehouse: On, or about, February 22, 2023, an inside flammable storage room had been built in the exit route leading from the warehouse to the exterior fire escape. This created a dead end corridor in the warehouse section of the building that exceeded fifty feet and a common path of travel of greater than 100 feet before employees reached the Production Area where there was access to other exit routes. A stairwell located in the warehouse did not meet the requirements of an exit route and was marked as such.

Machine Shop: On, or about, February 17, 2023, a series of black extension cords and relocatable power taps were used to energize battery chargers and a back-up power unit for the computer. D. Clean Room: On, or about, February 17, 2023, a Ridgid shop vac was attached to piping that allowed vacuum cleaner attachments to be used in the room. Flexible cord was used to provide power between the on/off switch and the outlet that energized the shop vac.

Electrical conduit was installed as a guardrail 14.5 inches in front of the face of the panel.
