(a) 11th Avenue Corporation, Inc., at 1112 Broadway, Denver, CO 80203: On and preceding January 4, 2021, the employer did not inform employees conducting housekeeping operations of the location of ACM and PACM in the building. A building survey identified multiple building materials that contained asbestos. Housekeeping and maintenance staff performed work where the asbestos containing materials were present. This condition exposed employees to an asbestos hazard.

Employees used chemicals for housekeeping. Chemicals in the workplace included, but were not limited, to the following: 1. Ecolab Peroxide Multi Surface Cleaner and Disinfectant; 2. Ecolab Heavy Duty Alkaline Bathroom Cleaner and Disinfectant; 3. Ecolab Pro Relaxing Spa; 4. Ecolab 66 Heavy Duty Alkaline Bathroom Cleaner and Disinfectant; 5. Linseed Oil; 6. Clorox Bleach.
