29 CFR  1926.451(g)(1):Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  Location:  Job site  Pump jack scaffolds lacked mid-rails and end rails.  An employee was injured and fell approximately sixteen feet from one of the scaffolds.

29 CFR  1926.451(b)(5)(ii):The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end.  Location:  Scaffolds at east and south ends of building.  Scaffold work platforms extended more than 18 inches past their support.
