29 CFR  1910.28(b)(11)(ii): The employer did not ensure that each flight of stairs having at least 3 treads and at least 4 risers is equipped with stair rail systems and handrails as described in 29 CFR 1910.28 Table D-2-Stairway Handrail Requirements.  a) Waste Water Treatment Area; On or about January 6, 2021 and at times prior to the employer exposed employees to fall hazards in that the employees were allowed to walk up and down fixed stairs with four or more risers that did not have a standard stair railing that included a top-rail and a mid-rail on the left hand side of the stairs that had exposed edges of four feet at the lowest riser to a waste trailer below.
