29 CFR  1926.652(d)(2):  Manufactured materials and equipment used for protective systems shall be used and maintained in a manner that is consistent with the recommendations of the manufacturer, and in a manner that will prevent employee exposure to hazards:   At a worksite located at the southwest corner of Tri-Park Way and Innovation Court in Appleton, Wisconsin, the employer did not use protective systems in a manner that was consistent with the recommendations of the manufacturer, and in a manner that will prevent employee exposure to hazards:  a)  The Pro-Tec Trench Shield (Model PRO4-816D, S# 16549) was stacked on a different manufacturer's trench shield (Efficiency Production Inc. Model MHXLDF-810, S# 127737) without the use of manufacturer approved stack connections as specified in the PRO4-816D's "Trench Shield Certification" (tabulated data sheet). This instance exposed employees to potential lateral movement of the stacked trench shield.  b)  The Pro-Tech Trench Shield (Model PRO4-816D, S#16549) was installed without adequate cave-in protection while employees conduced tie-in of storm/sewage water components in an approximately 14-feet deep excavation. A steel road plate and half-inch plywood sheet were placed along the northern, open end of the trench shield. This instance exposed employees to potential cave-in hazards.  c)  The Efficiency Production Inc. Trench Shield (Model MHXLDF-810, S# 127737), which was used for tying-in storm/sewage water components in an approximately 14 feet deep excavation, was not installed with manufacturer designed end plates to protect from cave-ins. A steel road plate was placed along the northern, open end of the trench shield, and two smaller steel road plates were placed along the southern open end of the trench shield. This instance exposed employees to potential cave-in hazards.
