29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) On or about January 8, 2021, employees were exposed to a fall hazard of 25-feet to the ground below.

29 CFR  1926.502(d)(21):Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service:  (a) On or about January 8, 2021 an employee was working from a balcony and was provided with a Werner Upgear H511202 green harness. The harness contained defects caused by the d-ring and cam buckles and other hardware having excessive corrosion and deformation exposing an employee to the hazard of falling 25-feet to the ground below.
