29 CFR  1910.147(c)(1):  The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  On or about January 11, 2021, the employer did not establish an energy control program consisting of energy control procedures, employee training, and periodic inspections to ensure employees performing servicing, maintenance and set up on Tarnow, Cincinnati, Sirco, Maxwell, LeBlond, other lathes and other mechanical equipment were protected from unexpected energizing, startup, or release of stored energy.  Machines and equipment used by the company are powered by electric, pneumatic, and hydraulic power.

On or about January 11, 2021, the employer did not ensure the operator(s) and other employees of the Cincinnati 26 inch lathe were protected by one or more methods of machine guarding from the hazards created by the point of operation and rotating parts of the machine. There was no chuck guard or guarded work zone to contain processed materials, fluids, parts, and tooling within the work zone.  c. On or about January 11, 2021, the employer did not ensure the operator(s) and other employees of the LeBlond 16 inch lathe were protected by one or more methods of machine guarding from the hazards created by the point of operation and rotating parts of the machine. The chuck guard was not properly used to contain processed materials, fluids, parts, and tooling within the work zone. The guard was observed in the up position while in operation.  d. On or about January 25,  2021, the employer did not ensure the operator(s) and other employees of the Tarnow  were protected by one or more methods of machine guarding from the hazards created by the point of operation and rotating parts of the machine.  The chuck guard was not properly used to contain processed materials, fluids, parts, and tooling within the work zone. The guard was observed in the up position while in operation.
