29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent:  Virtua Our Lady of Lourdes Medical Center  located at  1600 Haddon Avenue in Camden, NJ :   a) On or about 12/23/2020 the employer did not record a work related illness of a Food Service Worker who tested positive for COVID 19 on 12/16/2020 and had lost 15 lost work days on the OSHA 300 log or its equivalent.   b) On or about 12/28/2020, the employer did not record a work related illness of a Registered Nurse who tested positive for COVID 19 on or about 12/21/2020 and had lost 14 work days on the OSHA 300 log or its equivalent.  c) On or about 12/30/2020, the employer did not record a work related illness of a Registered Nurse who tested positive for COVID 19 on or about 12/23/2020 and had lost 12 work days on the OSHA 300 log or its equivalent.  d) On or about 1/19/2021, the employer did not record a work related illness of a Patient Care Technician who tested positive for COVID 19 on or about 1/12/2021 and had lost 13 work days on the OSHA 300 log or its equivalent.  NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
