Pool Room: The employer did not initiate and maintain frequent and regular inspections of the equipment used to prevent falls and accidents of up to 12 feet around the pool. On 24 September 2020, an employee fell through a barricade and fell 12 feet to the bottom of the pool sustaining multiple serious injuries.
