29 CFR 1926.102(a)(1): Eye and face protective equipment being used did not used meet the requirements specified in Z 87.1-1968:                                                                                                            The employer is failing to protect employees from eye injuries associated with operating pneumatic nail guns. This was most recently documented on or about January 13, 2021 at a residential construction jobsite located at 5408 West Madison Avenue, Lincoln, Nebraska. Employees were operating pneumatic nail guns while shingling a roof without the use of eye protection.

This was most recently documented on or about January 13, 2021 at a residential construction jobsite located at 5408 West Madison Avenue, Lincoln, Nebraska. Employees were observed working on a residential structure with a roof pitch greater than 4:12. The employees where installing shingling while working at a height of approximately 12 feet 2 inches measured from the ground to eave without fall protection.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards:  The employer is failing to train all employees on fall hazards they were exposed to. This was most recently documented on or about January 13, 2021 at a residential construction jobsite located at 5408 West Madison Avenue, Lincoln, Nebraska.  Employees were observed working on a residential structure with a roof pitch greater than 4:12. 29 CFR 1926.1053(b)(1): The employer is exposing employees to fall hazards where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  The employer is failing to protect employees from fall hazards associated with using ladders during residential construction. This was most recently documented on or about January 13, 2021 at a residential construction jobsite located at 5408 West Madison Avenue, Lincoln, Nebraska. The ladder used by the employees to access the upper landing surface of the residence was extended to a point approximately two feet below the top of the eave.
