29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a.  On or about January 12, 2021, at the above addressed jobsite, the portable ladder used to access the garage roof did not extend at least three (3) feet above the upper landing surface, thereby exposing employees to the hazards associated with falls.

On or about January 12, 2021, at the above addressed jobsite, an employee using a pneumatic nail gun was not wearing safety glasses, thereby exposing the employee to hazards associated with eye injuries.  Roman Bobak dba Roman Bobak was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1496170, citation number 1, item number 1, and was affirmed as a final order on April 16, 2021, with respect to a workplace located at 948 Western Avenue in New Lenox, IL.

On or about January 12, 2021, at the above addressed jobsite, employees performing residential roofing operations, ten (10) feet nine (9) inches above the ground, were not protected from falls, thereby exposing the employees to fall hazards.
