29 CFR 1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested prior to initial use of the respirator or annually refitted: a) Omni Manor, Inc. DBA Windsor House at Omni West Assisted Living located at 3259 Vestal Road, Youngstown , OH 44509 : On or about January 7, 2021, the employer did not provide a fit test to all employees required to wear respirators. The employer required employees to wear N95 filtering facepiece respirators to protect against the SARS-CoV-2 virus while providing care to suspected and confirmed positive COVID-19 residents. The Omni Manor, Inc. DBA Windsor House at O'Brien Memorial Healthcare Center was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134(f)(2), which was contained in OSHA inspection number 1476668, citation number 01, item number 001 B and was affirmed as a final order on December 21, 2020, with respect to a workplace located at 563 Brookefield Ave. SE, Masury, OH 44438.
