29 CFR  1926.102(a)(1):29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  On or about January 13, 2021, one employee was installing facia and siding with a staple gun to the exterior of the multifamily apartment building under construction without eye protection, exposing the employee to the hazard of being struck by flying objects.

29 CFR  1926.453(b)(2)(iv): Employee(s) were not standing firmly on the floor of the basket, but were instead sitting or climbing on the edge of the basket.  On or about January 13, 2021, an employee was observed climbing out of the raised aerial lift basket onto the roof of the building while still being connected to the basket, exposing the employee to the hazard of being pulled off the roof.
