On or about January 13, 2021, one employee was installing facia and siding with a staple gun to the exterior of the multifamily apartment building under construction without eye protection, exposing the employee to the hazard of being struck by flying objects.

On or about January 13, 2021, an employee was observed climbing out of the raised aerial lift basket onto the roof of the building while still being connected to the basket, exposing the employee to the hazard of being pulled off the roof.
