29 CFR  1910.261(b)(1): The employer did not ensure that the main power disconnect switch or valve, or both, controlling its source of power or flow of material, was locked out or blocked off with padlock, blank flange, or similar device before conducting any maintenance, inspection, cleaning, adjusting, or servicing of equipment (electrical, mechanical, or other) that required entrance into or close contact with the machinery or equipment:   On or about January 13, 2021, the First Dryer Section was not shut down or locked out at the power source when employees physically accessed the Press Pit Platform during a paper break. Employees were exposed to rotating and moving parts of the First Dryer Section.

29 CFR  1910.147(f)(3)(i):  A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew, craft, department or other group:  On or about January 13, 2021, employees accessed the Press Pit Platform during a paper break while the First Dryer Section was operating for the purpose of cleaning the platform. A group lockout procedure was not utilized, exposing employees to making contact with moving parts of the First Dryer Section.

When wet processes are used, drainage was not maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats were not provided:  On or about January 14, 2021, 6th hand employees were working in the basement below the paper machine with standing water on the working surface.
