29 CFR 1910.176(a): Where mechanical handling equipment was used, sufficient safe clearances was not allowed for aisles, at loading docks, through doorways and wherever turns or passage were made. Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways were not appropriately marked: Employee(s) engaged in operating a powered industrial truck in the west bay passageway (outside of the breakroom door) at the facility located at 815 E. 6th Street, Le Roy, Kansas were exposing other workers to struck-by hazards. The west bay passageway (outside of the breakroom door) was not appropriately marked to indicate safe clearances for powered industrial trucks.
