At the worksite:  On or about 1/14/2021, the employer allowed employees to use a scaffold component that was not capable of supporting it's own weight and at least 4 times the maximum intended load, in that the employer used one common 2" by 8" board for the scaffold planking, exposing the employees to a fall hazard of approximately 24 feet.

At the worksite On or about 1/14/2021, the employer allowed employees to use a common wood plank as a working platform. The wood plank measured approximately 16' 1" in length, and extended approximately 2 feet over the scaffolding supports at each end, which was not designed so that the cantilevered portion of the platform was able to support employees without tipping.

At the front of the building  On or about 1/14/2021, the employer did not ensure that employees tasked with working from a ladder-jack scaffold system at an approximate height of 24-feet above the lower level, were trained to recognize the hazards associated with this type of scaffold, in that the height exceeded the 20-foot limit, fall protection was not used and the platform plank did not meet scaffold grade criteria.
