29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) On or about January 12, 2021, the employer did not instruct or provide instruction to employees in the recognition and avoidance of unsafe conditions and the regulations applicable to their work environment to control or minimize the hazards associated with working on and around energized electrical circuits. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.416(a)(1):No employer shall permit an employee to work in such proximity to any part of an electric power circuit that the employee could contact the electric power circuit in the course of work, unless the employee is protected against electric shock by deenergizing the circuit and grounding it or by guarding it effectively by insulation or other means: a) On or about January 12, 2021, employees were exposed to energized electrical parts while working on a 120/240 Volt 400 AMP Main Distribution Panel that was not de-energized prior to the performance of their work. No abatement certification or documentation required for this violation.
