29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. Guardrail systems were not installed before the scaffold was released for use by employees other than erection/dismantling crews: a. North pumpjack, west side of building: On or about January 15, 2021, employees were exposed to a fall hazard greater than 10 feet, while attaching siding from a pumpjack scaffold, without end rails attached. b. South pumpjack, west side of building: On or about January 15, 2021, employees were exposed to a fall hazard greater than 10 feet, while attaching siding from a pumpjack scaffold, without end rails attached.

29 CFR 1926.453(b)(2)(v):A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift. Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e): On or about January 15, 2021, an employee was exposed to a fall hazard, while performing cornice work from a Genie aerial lift, without using fall protection.
