What this inspection record means

OSHA opens inspections for many reasons — routine scheduling under a national or local emphasis program, an employee complaint or referral, or a follow-up after a reported injury. 29 CFR 1926.651(k)(1):Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems: (a) Fairwind Properties LLC at 10781 Arapahoe Rd, Erie, CO: On and around January 15, 2021, the exposing, creating, and correcting employer did not ensure that daily inspections of an excavation, which was approximately 10 feet wide and 18 feet long, which had a maximum measured depth of approximately 20.5 feet, and which had near vertical or undercut earth walls, were made by a competent person for evidence of a situation that could have resulted in a hazardous condition. Two employees were working in two stacked eight feet high two-sided cave-in shield systems without endplates that created an approximate 2.5 foot long section of the earth wall unshielded at the top of the excavation above the shield system. These conditions exposed the employees to cave-in hazards from open sides of the two stacked shield systems and the unshielded earth wall at the top of the excavation above the shield systems.
