Site - On or about January 14, 2021, an employee installing sheathing on a residential structure was exposed to falling approximately 19 feet to the ground, and the employer failed to ensure that an appropriate fall protection system was used.  b. Site - On or about January 14, 2021, employees working on the second floor of a garage were exposed to falling approximately 12 feet to the ground through unguarded wall and floor openings, and the employer failed to ensure that an appropriate fall protection system was used.
