29 CFR  1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  a) On or about January 12, 2021 at the worksite located at 130 Morris Creek Drive, employees working in a benched trench were exposed to engulfment hazards when the employer failed to ensure the that they had a safe means of egress. The employer provided a ladder, however the ladder was not long enough to reach the top of the excavation, causing the employees to have to climb up the benches to exit the excavation.  b) On or about January 13, 2021 at the worksite located at 130 Morris Creek Drive, employees working in a benched trench were exposed to engulfment hazards when the employer failed to ensure the that they had a safe means of egress.The employer provided a ladder, however the ladder was not long enough to reach the top of the excavation, causing the employees to have to climb up the benches to exit the excavation.
