Employee engaged in painting and coating activities in the main shop at the paint booth was exposed to health hazards in that the employer had not determined, through a medical evaluation, the employee's ability to use a respirator before the employee is fit tested or required to use a respirator.  29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

29 CFR 1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested prior to initial use of the respirator:  Employee was exposed to health hazards.  Employee engaged in painting and coating activities in the main shop at the paint booth was exposed to health hazards in that the employer did not provide a fit test to all employees required to wear respirators. Employees are required to wear Half Face-piece North 7700-30M Part #7700-11M respirators when painting.  29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Employees were exposed to amputation and struck-by hazards. Employees engaged in production activities in the main shop at the following equipment areas were exposed to amputation hazards in that machine guarding were not provided:  a) At the blue Drill Press machine #222. b) At the Powermatic Drill Press machine #2015, Model #1150, Serial #5-S-3230. c) At the Delta Drill Press. d) At the Walker Turner Drill Press Serial #1944. e) At the Delta Drill Press machine #1916. f) At the Delta Drill Press.  29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

Employees engaged in material handing activities using the Clark and Nissan forklifts were exposed to struck-by hazards in that comprehensive forklift training was not provided prior to permitting forklift operation.  29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

Employees engaged in material handing activities using the Clark and Nissan forklifts, were exposed to struck-by hazards in that an evaluation of each powered industrial truck operator's performance was not conducted:  a) An employee operated forklifts while the person's performance had not been evaluated since 2/25/2016. b) An employee operated forklifts while the person's performance had not been evaluated since 9/12/2013. c) An employee operated forklifts while the person's performance had not been evaluated since 8/4/2011.  29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.
