29 CFR 1910.22(a)(3): 29 CFR 1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice. (a) Jobsite, Oakmont Country Club, Oakmont, PA: On or about January 8, 2021 the walking/working area was not free from protruding objects or obstruction during the felling of a tree. The area where employees were positioned with a guide rope was directly next to a drainage gully system built with large rocks thereby not free of hazards or objects. An employee lost their footing when the rope went slack and the tree began to fall and they tried to move from the path of the tree. The employee fell, face first, onto the rocks sustaining head trauma.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: (a) Facility, 709 Kiski Park Drive, Apollo, PA: On or about January 8, 2021 an employee sustained injuries that resulted in an in-patient hospitalization and the employer did not report the work-related incident to the local Occupational Safety and Health Administration office.
