29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   At this commercial building: On January 19, 2021, and at times prior thereto, employees were exposed to a fall hazard greater than 6 feet to the ground below while performing demolition activities from the top of a roof without a fall protection system in place.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   At this commercial building: On January 19, 2021, and at times prior thereto, employees without fall protection were exposed to a fall hazard greater than 6 feet to the ground below while performing demolition activities without fall hazard recognition training.

29 CFR  1926.1053(b)(1):Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:  At this commercial building: On or about January 19, 2021, employees were exposed to a fall hazard, when using a ladder that was less than 3 feet above the upper landing surface to access the roof.
