29 CFR  1910.30(a)(1): (a) Fall hazards.  (1) Before any employee is exposed to a fall hazard, the employer must provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart.  Employers must ensure employees are trained in the requirements of this paragraph on or before May 17, 2017.  (a)  On or about January 26, 2021, the employer had not provided training on all of the different types of fall protection harnesses and lanyards that employees were required to use while operating the Yale turret truck, Raymond reach truck, Toyota order pickers, Raymond order pickers, Jungheinrich order picker when picking orders in the warehouse.

29 CFR  1910.151(c):  Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use. [63 FR 33450, June 18, 1998]  (a)  On or about January 26, 2021, the eyewash station in the battery charging area, where employees were exposed to sulfuric acid (corrosive) when they added water to the batteries of powered industrial vehicles, was blocked by the parked floor scrubber, two metal carts, and a wooden pallet containing two large floor fans.

(a)  On or about January 26, 2021, powered industrial vehicle operators did not receive training that included all of the different types of fall protection systems, such as fall protection harnesses:  Honeywell (yellow), Honeywell Miller Titan II (orange), Miller (yellow), Python (green), Falltech (blue), and fall protection lanyards: Maynard II (green), Titan Miller by Honeywell (yellow), Falltech (blue), Buckingham Mfg. 29 CFR  1910.178(l)(4)(ii)(A):  Refresher training in relevant topics shall be provided to the operator when the operator has been observed to operate the vehicle in an unsafe manner.  On or about January 21, 2021, the employer did not conduct refresher training for PIV operators who were observed to operate the vehicle in an unsafe manner.  (a)  On 1/8/2021, an employee was coached, instead of receiving refresher training, for not using the horn of their PIV at intersections and ends of aisles in the warehouse.  (b)  On 12/22/2020, an employee was coached, instead of receiving refresher training, for not stopping their PIV and using the horn at the end of aisles in the warehouse.

29 CFR  1904.7(b)(3):How do I record a work-related injury or illness that results in days away from work? When an injury or illness involves one or more days away from work, you must record the injury or illness on the OSHA 300 Log with a check mark in the space for cases involving days away and an entry of the number of calendar days away from work in the number of days column. If the employee is out for an extended period of time, you must enter an estimate of the days that the employee will be away, and update the day count when the actual number of days is known.  On or about January 26, 2021, the employer did not correctly enter the following work-related injuries or illnesses on the OSHA 300 form for the calendar year 2020;  (a)  On December 2, 2020, an employee working as an order picker on the pack/ship line experienced back pain after lifting a heavy box.  The employee received four days away from work and five days of restricted duty, but the numbers of days were not recorded in columns (K) and (L) on the OSHA 300 Log for the calendar year 2020.    (b)  On November 28, 2020, an employee working as a bulk seasonal picker at the bulk shipping station east dock experienced a contusion to the left toe causing nail damage after a box fell on their foot.  The employee received five days away from work and ten days of restricted duty, but the numbers of days were not recorded in columns (K) and (L) on the OSHA 300 Log for the calendar year 2020.

29 CFR  1910.132(d)(2):  The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.  (a)  The employer did not certify in writing that they had completed a workplace hazard assessment to determine what personal protective equipment would protect employees form the hazards identified in the workplace.
