29 CFR  1910.213(c)(1): Circular handfed ripsaw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut:  a) On or about January 12, 2021, in the fabrication area, an employee operating the Grizzly Industrial Inc., G1023Z 10# Table Saw Z Series, Serial number 035441, was exposed to laceration and amputation hazards when the blade was not guarded.   b) On or about January 12, 2021, in the fabrication area, an employee operating the Delta Unisaw 10" Tilting Arbor Saw, Serial number 93D92235, was exposed to laceration and amputation hazards when the blade was not guarded.

29 CFR  1910.213(c)(2): Hand-fed circular ripsaw(s) were not furnished with a spreader to prevent material from squeezing the saw or being thrown back on the operator:  a) On or about January 12, 2021, in the fabrication area, an employee operating the Grizzly Industrial Inc., G1023Z 10# Table Saw Z Series, Serial number 035441, was exposed to struck-by hazards when the hand- fed circular ripsaw was not furnished with a spreader to prevent  wood from being thrown back at the operator.   b) On or about January 12, 2021, in the fabrication area, an employee operating the Delta Unisaw 10" Tilting Arbor Saw, Serial number 93D92235, was exposed to struck-by hazards when the hand- fed circular ripsaw was not furnished with a spreader to prevent  wood from being thrown back at the operator.

29 CFR  1910.213(c)(3): Hand-fed ripsaw(s) did not have non-kickback fingers or dogs so located as to oppose the thrust or tendency of the saw to pick up the material or to throw it back toward the operator:  a) On or about January 12, 2021, in the fabrication area, an employee operating the Grizzly Industrial Inc., G1023Z 10# Table Saw Z Series, Serial number 035441, was exposed to struck-by hazards when the hand-fed ripsaw did not have kickback prevention methods installed.     b) On or about January 12, 2021, in the fabrication area, an employee operating the Delta Unisaw 10" Tilting Arbor Saw, Serial number 93D92235, was exposed to struck-by hazards when the hand-fed ripsaw did not have kickback prevention methods installed.

29 CFR  1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  a.	On or about January 19, 2021, in the establishment, the employer failed to develop and implement a hazard communication program in accordance with 29 CFR 1910.1200(e)(1) that would describe or include at least the following:  1) Requirement for labeling of containers of hazardous chemicals; 2) Safety data sheet availability; 3) Training of employees; 4) A complete list of hazardous chemicals known to be present in the workplace; 5) Methods to inform employees of the hazards of non-routine tasks; and 6) Methods to inform other employer(s) of material safety data sheet availability; the labeling system and any precautionary measures to protect employees.  Employee exposure included, but was not limited to quinacridone and benzophenone.
